MediCreole provides Creole/English discharge navigation services to help reduce readmission rates for Haitian patients. Our service includes explaining discharge plans, scheduling follow-ups, and providing medication support to prevent unnecessary ER visits.
By using our service, you agree to these Terms of Service ("Terms"). Please read them carefully before using our discharge navigation services.
Service Description
Our discharge navigation services include:
- Explaining discharge instructions in Creole or English
- Scheduling and coordinating follow-up appointments
- Providing medication education and support
- Connecting patients with appropriate healthcare resources
- Monitoring recovery progress and adherence to care plans
- Facilitating communication between patients and healthcare providers

Service Availability
Our discharge navigation services are available during business hours (Monday-Friday, 8 AM - 6 PM EST). We strive to provide timely support but cannot guarantee immediate availability for all requests.

We are committed to protecting your privacy and maintaining the confidentiality of your health information in accordance with HIPAA and applicable privacy laws. Please review our Privacy Policy for detailed information about how we collect, use, and protect your information.
Limitation of Liability
MediCreole provides navigation and support services only. We are not responsible for:
- Medical outcomes or treatment results
- Decisions made by your healthcare providers
- Delays in receiving medical care
- Changes in your health condition

In no event shall MediCreole be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services.
